Payment Policy.
Self-pay (out-of-network) clients:
I will provide you with a superbill suitable for you to submit to your insurance. The superbill (which will also serve as a payment receipt) will be coded appropriately to the level of service provided during the visit. You agree to pay me at the time of the visit (cash, check, credit card, or FSA).
Insurance (in-network with TLN or Wildflower) clients:
My initial visit includes 2 weeks of follow up support by secure messaging or email. Continued support is available for a monthly fee of $40 via secured messaging through the client portal. These fees are elective and not eligible for insurance reimbursement.
Using Paypal:
Udderly Love Lactation uses Paypal to process payments made electronically using a credit card or fund transfer. Payments may be made electronically using a credit card or fund transfer. Paypal is a secure and private platform that meets the high standards of HIPAA and the banking industry. However, Paypal may send, automatically or per your request, email or text message receipts that reveal personal health information such as the date and type of lactation visit.
Cancellation policy:
Please understand that you are responsible for all charges associated with this visit. If you cancel with less than 24 hours notice, your credit card on file will be charged $100. For example, if your appointment is on Wednesday at 9:00am I need to hear from you BEFORE Tuesday at 9:00am. You are allowed to reschedule up to 4 hours before your scheduled visit. To avoid this charge, you must notify us the day before your appointment. If you need assistance with rescheduling, please contact me directly at (470) 931-9287.
Starting September 1, 2024, a travel fee will be required for all home visits.
Travel Fees for Home Visits
Travel fees are already included in the consultation price for all home visits.
General Travel Fee:
$50 for locations within a 20-minute radius of Decatur
For clients using TLN or Wildflower coverage, the $50 travel fee is still required and is included in your total cost.
This fee is not covered by insurance.
It will be applied to your invoice after scheduling and charged to the card on file on the day of your appointment.
